Based on the extension that I have reviewed, this extension has a special installation procedure that consists in adding plenty of files into the VirtueMart directory and also adding "vm_%" tables.
There is no specific joomla extension created except a module and a plugin that is added.

As this VM Affiliate just add things in the VirtueMart then you can just use the JMS Tool menu and the "VirtueMart" component definition to automatically install the Affiliate that is included in VirtueMart.

Concerning the additional module and plugin, you can just define them.
We will add their definition in a next release of JMS Multisites patches.
