YES JMS can create DB and also create users when you provide to it a MySQL root login and password in the "multisites.cfg.php" files as explained in user manual.

So the key element is to provide a MySQL user that has the permission to create such dynamic DB.
When the login and password you provide does not allow such permission, JMS try to use the alternative MySQL root login and password you have provided in the configuration file.

So in your case, if you have not configured the JMS configuration file with such MySQL root login and password, it uses the test5_jms and its password to try create the DB.
If this "test5_jms" has no MySQL permission to create the DB, the operation will fail.

Im still not create new DB using Template, even if i put now the MySQL root username and root password.

Is there a related on phpMyAdmin and MySQL version? so that JMS cant create new DB

Here my version:

php - 5.2.6
MySQL client version: 4.1.22
Server version: 5.0.81-community
phpMyAdmin - 2.11.9.5

I also use "localhost" as a DB host name, both on template and multisite.cfg.php.

Thanks,

No this is not related to PYPMyAdmin or whatever.
JMS uses the MySQL login and password to perform the operation.
If the permission does not allow the user to create such DB and/or users, the creation may fail.

If the error message does not provide you a clear reason, the only solution is to debug the db creation on your system.
If you want that we debug your system, this is possible with our billable support that you can order on www.jms2win.com/download?page=shop.produ...35&category_id=1
