Hi Edwin,

Thanks for the response. I change templates to black and now it shows a success message. I guess the default template does not. I did configure like you said and it is working!! Very nice. However I dont need site prefix at all in the form. I can just disable it right?

In the advanced setting, you can decide which field to show or hide.
So up to you to display the field that you want.

in my template for Site ID: I have {site_alias}. This is fine if they just enter 'mysite' and then I can just add '.com' in the domains with:

www.{site_alias}.com

However, this is not ideal because it's not certain that all domains will be .com. Some might be .net, .co, etc. So how to handle this?

Site alias is a wider field.
So you can enter a full domain that you could assign to the Site ID.
After that, you could extract peace of the {site_id} to get element of the path.
ie

www.domain.com
3 2 1

If you want to extract the "domain" word, you can use the {site_id-2}
for the "com", use {site_id-1}
